VA Palo Alto Health Care System has an immediate opening for an Inpatient Clinical Pharmacist at our Palo Alto Division. The Inpatient Pharmacy provides pharmaceutical services to our 300-bed Acute Care (ICU, Medicine, Surgery), Rehabilitation (Spinal Cord Injury, Poly Trauma), Psychiatry, and SubAcute/Hospice programs with centralized operation for Unit Dose, IV Admixture and Discharge medication preparation and decentralized clinical work. We have a fully integrated computer system including online provider order entry, documentation of medication administration using barcode technology, and automation of the unit dose distribution system. This pharmacist will primarily provide comprehensive clinical and distributive pharmacy services to the Acute Medicine units, but will also provide coverage to other areas on an as needed basis.
Specific responsibilities of this clinical pharmacist include:
Clinical Responsibilities
* screening/reviewing medication orders for appropriateness of drug therapy and verifying the orders
* making clinical interventions
* monitoring high alert drugs as appropriate
* participating in interdisciplinary rounds and other appropriate meetings
* performing medication reconciliation upon discharge (proactively assessing discharge medication needs, verifying discharge prescriptions, counseling)
* participating in emergency code and pre-emergency situations as assigned
* documenting clinical work in appropriate progress notes
Education/Training
* providing education to patients and caregivers about medications and side effect management
* providing education to physicians, nursing and pharmacy staff
* precepting pharmacy residents and students on clinical rotation

Basic Requirements:
* U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
* Education: Applicants must meet either A or B.
A) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. NOTE : Prior to 2005 ACPE accredited both baccalaureate and Doctor Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy. TRANSCRIPT(S) MUST BE SUBMITTED AT THE TIME OF APPLICATION. CONFERRAL OF DEGREE(S) SHOULD BE PRESENT ON TRANSCRIPT(S). UNOFFICIAL TRANSCRIPT(S) ARE ACCEPTABLE.
B) Graduates of foreign pharmacy degree programs meet the education requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). TRANSCRIPT(S) MUST BE SUBMITTED AT THE TIME OF APPLICATION. CONFERRAL OF DEGREE(S) SHOULD BE PRESENT ON TRANSCRIPT(S). UNOFFICIAL TRANSCRIPT(S) ARE ACCEPTABLE. YOU MUST ALSO SUBMIT A COPY OF YOUR CURRENT FPGEC CERTIFICATE.
* Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. YOU MUST SUBMIT A COPY OF YOUR LICENSE WITH YOUR APPLICATION.

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.

Grade Determinations:Completion of the equivalent of one (1) year of creditable experience equivalent to the next lower grade level, which is directly related to the position to be filled OR completion of an ACPE-accredited Pharm.D. program.

Demonstrated Knowledge, Skills, and Abilities: Experience is professional experience that equips the applicant with the following knowledge, skills and abilities:

  • Knowledge of professional pharmacy practice.
  • Ability to communicate in-person and in writing to both patients and health care staff.
  • Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
  • Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Physical Requirements: This position requires moderate lifting and carrying, 15-44 pounds; use of fingers and both hands required; walking and standing up to 4 hours daily; both legs required; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other; depth perception; ability to distinguish shades of colors; hearing without aid; working closely with others; working alone; protracted or irregular hours; and unusual emotional strain.
