Distributes, controls and monitors drug use in assigned areas in the hospital and educates health professionals and patients regarding that drug use.
POSITION DUTIES (ESSENTIAL FUNCTIONS, INCLUDE % OF TIME)
MEDICATION ASSESSMENT - 30% Interprets, evaluates, and monitors patient medication orders in a timely manner using the patient medical record and or pharmacy profile. Dispenses medication in an accurate and timely manner following hospital formulary guidelines.
Inpatient Setting Participates in Code Blue and other emergency situations to ensure adequate drug supply and appropriate drug use.
Out Patient setting: Works with hospice and other outpatient services to ensure adequate drug supply and appropriate drug use. Demonstrates high tech skill level utilizing a variety of pharmaceutical competencies including, but not limited to calculations, aseptic technique, cytotoxic drug properties, and vancomycin / aminoglycosides.
DOCUMENTATION - 25% Maintains patient and other records using pharmacy computer and manual systems to ensure timely and accurate patient billings and compliance with regulatory agencies. Facilitates maintenance of adequate supplies by logging need in “Want Book”.
COLLABORATION - 15% Works effectively with physicians and other healthcare providers to provide high quality, cost-effective use of pharmaceuticals.
Inpatient Setting Provides drug information to the health care team by verbal communications, inservices, memos and newsletters to ensure appropriate prescribing and administration of medications.
Inpatient Setting Provides drug information to patients by verbal and written instructions to ensure understanding of prescribed medications.
PROBLEM MANAGEMENT - 15% Recognizes and resolves inappropriate orders for medications using amassed technical knowledge and effective communication with healthcare team. Prioritizes daily work load demands by balancing dispensing requirements with time requirements for the resolution of questionable orders. Provides work direction to technicians, interns and externs. Attains professional growth by attending or participating in at least 12 hours of continuing education programs annually. Performs special projects and other activities as assigned to increase and improve level of service, skill level, and productivity.
Demonstrates ability to remain calm and provide appropriate care in emergency situations.
PROCESS IMPROVEMENT - 15% Assists in design, promotes and actively participates in department-based P.I. Program. Professional role model for all staff, demonstrating high technical ethical / legal professional pharmacy practices. Delegates to non pharmacy personnel in accordance with state Board of Pharmacy and hospital policy. Conduct individualized orientation to new employees and provides on going coaching to all employees. Identify clinical and operational needs of department and coordinate with peer teams and management to develop and implement interdisciplinary action plan. Actively supports management in establishing standards of practice, noting non-compliance and participating in corrective actions plans.

Castle Rock Adventist Hospital is a 50-bed acute care hospital with a possible 60-beds shelled for future growth. The Hospital will have a separate central plant and will be approximately 212,000 square feet. The hospital will be four stories with an intensive care unit, medical/surgical unit, women’s unit, and a full complement of hospital services including a pharmacy, laboratory, three operating rooms, two dedicated cesarean-section operating suites, nuclear medicine, women’s imaging center, and a combination cardiology/interventional radiology suite. Additional growth on the hospital campus includes a 60,000‐square‐foot medical office building and extensive site development.
